Information for Youth Registering Alone

Our registration system is based on the assumption that the leader of each family group is an adult. If you are a Youth (12 through 17) registering alone, then you must submit your registration together with the required Guardian Forms. You can download these forms here:

• MJAA Guardian Policy and Form
• MJAA Parent or Guardian Consent Form
• MJAA Guardian Affidavit
• MJAA Rabbi or Congregational Leader Affidavit
• Adobe Acrobat Reader (PDF viewer, if needed)

Please download and read these forms. Instructions for filling in each form are contained in the form. Fill out and either Fax the forms (if using a credit card) to (407) 366 4138, or mail your forms with payment to:
